I can’t fill complete all of the fields on the case identification spreadsheet – will it still be accepted?
The case identification spreadsheet has been amended and will now be sent out in two stages.
Stage 1: This will ask for a limited amount of information to identify all young people who fit the study criteria. The data collection fields requested will include:
• Trust/Health Board/GP practice name
• Hospital name (where applicable)
• NHS number
• Hospital number
• Date of birth
• Sex
• Included condition with ICD10 code or Snomed code where available
Stage 2: This will ask for further information on up to fifteen young people. The data collection fields requested will include the details of:
• The clinical teams providing care to the young person
• Previous attendances to the Trust/Health Board
• Other care providers – primary, community, physical and mental health care
Spreadsheets need the key fields to be completed to be accepted. If you are unable to complete the key fields (highlighted with an *) please contact the NCEPOD office to discuss this further.
Do I need to identify young people via the school nursing service?
No, you do not need to identify young people via the school nursing service. However, if we select a young person for inclusion in the study who receives care via the school nursing service, we may send a clinician questionnaire for completion to that service where we can identify their details.
This organisation is a specialist centre and does not treat young people with any of the conditions listed for inclusion in the study. Do we still have to participate?
The patient identification spreadsheet will not need to be completed. However, if a young person is reported to us via another primary, community, or physical healthcare organisation and they are subsequently identified as also receiving care in your organisation, the relevant healthcare team will be sent the clinician questionnaire to complete, and they will be asked to return this alongside copied extracts of the case notes. An organisational questionnaire will also be sent for completion.
This organisation is a mental healthcare provider. Do we still have to participate?
The patient identification spreadsheet will not be sent for mental healthcare providers for completion. If a young person is reported to us via a primary, community, or physical healthcare organisation and they also have a mental health condition, the relevant mental healthcare team will be sent the clinician questionnaire to complete, and they will be asked to return this alongside copied extracts of the case notes. An organisational questionnaire will also be sent for completion.
This organisation is an independent healthcare provider. Do we need to participate in the study?
Independent providers of care only need to participate in the study if they provide transition services. If you work in an independent hospital and think your site should be participating, please get in touch at transition@ncepod.org.uk
In Spreadsheet B, does Column G (Please list all of the conditions the patient has*) require all conditions the patient has diagnosed with?
Column G asks for all conditions the patient was receiving care for during the study period.
In Spreadsheet B, does Column H (Please list the clinical team(s) responsible for the care of this patient in this organisation (specialty)* apply to teams only within the study period?
Column H is for all clinical teams who provide ongoing care for the patient, so this can be before the study period.
What should I do if when filling the Organisational Questionnaire, I would like to elaborate on the questions asked?

What if some questions on the Questionnaire apply to some sites within the organisation, but not others?
For this study the questionnaire has been designed to be completed at a Trust/Health Board level rather than at a hospital level. To upload the questionnaire to the online system, it needs to be assigned to a hospital site. Where there are multiple sites in your Trust or Health Board, we have selected one site to upload the questionnaire to, but please do complete this questionnaire in relation to the services provided by your Trust or Health Board as a whole.
How can I get involved in the parent/carer survey?
We've received a clinician questionnaire, but it is not addressed to the right specialty. What should I do?
Please contact the NCEPOD office so we can amend our records and reissue the questionnaire where applicable.

What kind of case notes are we requesting?
• Notes requested relating to transition include:
• Clinic letters
• Discharge summaries
• Transition documentation (including ‘Ready Steady Go’ and Transition plans)
• General MDT notes (all relevant health care professional MDT notes including those from allied health professionals)
• Education, Health and Care Plans (where available)
• Moving on Passport/Transition passport
• Care plans
• Treatment escalation plans
If a young person is seen in multiple clinics/by multiple specialties within the same organisation, notes are requested from all services. We are looking at ongoing care for this study, rather than a specific admission for this study, we do not require all notes from the admission, only what is listed in the above.
For what period are we requesting case notes?
We are requesting noted from the patient’s 13th birthday until the end of the study period (31st March 2021). If a young person is seen in multiple clinics/by multiple specialties within the same organisation, notes are requested from all services.
