  • What is my username?
    • Your username is your work email address.
  • Local reporter: What do I do if I have forgotten my password?
    • If you have forgotten your password, you can press the password reset button. If that fails, please call the NCEPOD office and we can resend the activation link.
  • The questionnaire website is not working on my internet browser.
    • Please ensure that you are using Google Chrome or Microsoft Edge as these internet providers are compatible with the online software. If you continue to experience problems please contact the NCEPOD office.
  • Who completes the organisational questionnaire?
    • Local Reporters can invite the appropriate personnel to complete the questionnaire, by entering the relevant clinician(s) email address on the invite screen. The clinician should then receive an email containing a link to complete the questionnaire. This link remains active for 2 weeks.
      Multiple personnel can be invited to complete the questionnaire, but it is recommended that only one person should make amendments to the questionnaire at one time.
      There is an automatic auto-save function, but please remind clinicians to press save on completion of a section before moving forward.
      NB: Even if a clinician has previously completed an organisational questionnaire or clinician questionnaire, to complete the current organisational questionnaire, they must access the questionnaire via the invitation email link.

  • How can I see who has worked on the questionnaire?
    • Local reporters can log in and see an audit trail which shows who has last accessed the questionnaire.
  • Can I have a paper copy of the questionnaire?
    • From within the online system, the organisational questionnaire can be downloaded as a PDF which displays the currently answered questions and this can be circulated to colleagues. If the organisational questionnaire is being completed on paper, the Local Reporter will need to enter the information online and submit the questionnaire. NCEPOD no longer accepts paper questionnaires.
  • How can I edit a questionnaire?
    • The pencil icon allows changes to be made to the questionnaire.
  • Can the invited clinician submit the questionnaire?
    • No. The Local reporter retains executive control in submitting the questionnaire when it has been completed. Only press submit when there are no further changes to be made to the questionnaire.
  • Can I be added as a Local Reporter for this study as I will be helping with dissemination of questionnaires?
    • Only contacts who are working on multiple NCEPOD studies are appropriate to be added to the online questionnaire system, and in these cases we need to receive a supporting written request from the current LR.
