A copy of the clinician questionnaire can be downloaded here. But please note that this questionnaire should be submitted via our online questionnaire submission system. For clinicians who received a request, please speak to your local contact (named in the invitation email) or contact us at 020 7251 9060 or bowelobstruction@ncepod.org.uk for more details.
I am a clinician who has received a request for a questionnaire to be completed; however, this patient did not have a bowel obstruction; this patient was miscoded, should I complete the questionnaire anyway?
No, please call the NCEPOD office on 020 7251 9060 or email bowelobstruction@ncepod.org.uk and/or contact your local Contact (named in the invitation email) to exclude the case from the study. Please give details of the actual diagnosis for our records.
I have received more than one request to complete a questionnaire, is this correct?
We have limited it to a maximum of 2 per clinician. However, if you are having problems, please contact your local contact (named in the invitation email) or contact us at the NCEPOD office 020 7251 9060 or email bowelobstruction@ncepod.org.uk
This patient did not die as a result of their bowel obstruction, should they still be in the study?
Yes, this study is looking at all patients who had a bowel obstruction and were admitted to hospital between: Monday 16th April - Sunday 13th May 2018, regardless of outcome.
I have received the link to complete a questionnaire, but I was not the operating surgeon for this patient, should I complete the questionnaire?
We have sent the questionnaire to the named admitting clinician from the hospital records. You should be able to complete the questionnaire using the case note record. However, if you feel that there is someone at your hospital who is more appropriate to complete the questionnaire, please contact your local contact or get in touch with us at the NCEPOD office 020 7251 9060 or email bowelobstruction@ncepod.org.uk
I received an email from : ncepod-questionnaires@sunstonetechnology.com with a link to register my details and complete a questionnaire. What personal information of mine will be recorded and why is this necessary?
Your name and email address will be held by NCEPOD and Sunstone Technologies for the purpose of allowing you to log on to the system and to allow us to reset your password should you require to do so. Your name and email address will be held securely for duration of the study after which time it will be destroyed and never passed on to any third party. The content of the questionnaire will only be held by NCEPOD.
